In November 2016 the programme introduced Champions in four residential homes in Sutton, this included champions for falls and champions for end of life care. This pilot ran until July 2017 and has been rolled out to nursing and residential homes.

The aim of the champions was to upskill two care staff to become competent in falls and end of life care. They developed their knowledge by working with the Supportive Care Home Team (in end of life care, part of the Care Home Support Team) and Falls lead from the community services and promoted best practice, introduced new initiatives to enhance the residents' independence and improve their health and wellbeing. The champions were encouraged to cascade information to their peers and recognise residents who required input from a healthcare professional. The falls champion monitored monthly falls and applied falls prevention strategies.

This pilot was ran alongside the pilot of the Wellbeing and Health Rounds in these same four homes.
