​​Enhanced Nursing Role Models in Nursing Homes

The enhanced nursing role model was piloted by the vanguard team across six nursing homes in Sutton in November 2015.  The initiative was designed to deliver a more proactive, preventative approach to care for residents to identify any potential health issues earlier and improve data capture of the state of residents’ health to ensure they continue to receive the most appropriate care for their specific health needs.

The pilot was undertaken for nine months (until July 2016) to test a new model of care in nursing homes - Delivering dedicated, proactive general practitioner input to residents and providing support for the GP from a care coordinator (registered nurse) in each home. A full evaluation was done upon completion of the pilot, and you can read more about the results in the final report.
